If I drilled a hole through a baby pumpkin and filled it with a toothpick and kept adding larger cylinders as it grew, then removed it when it's big enough to not fill itself in, could I grow a pumpkin with a hole through it?

Well I had a pumpkin that had a blossom split that put on an extra 200 pounds so I am sure you could keep it going at least for a while. I don't know if you would be able to get it all the way through the season though.

Just thinking in my head (bad idea) I wondered a few times if a pumpkin broke through to the cavity but was in some type of sealed box filled a inert gas, would it survive for quite awhile, the outside of the pumpkin would need to be sterile too, I guess ,in my head.

This is down the rabbit hole but baitman I think you have it. Theoretically, if you put gas ports on the box so that you could completely purge the chamber of oxygen, and backfill with nitrogen or argon, you could remove the reactive species that cause rot.
