how deep do you test the soil temperature? and what is the ideal temp to plant

I think most growers will agree that cold soil temperatures do more harm than any advantage a grower may think they are getting by planting early.
In my growing region (Pacific Northwest) we like to have a minimum soil temperature (at 6" depth) to be 60 degrees but preferred at 64-65 plus before I put my seedlings in the ground.
